How would I upload a proxy to my website?

Okay, I brought a domain from namecheap.com & I have got hosting. What I was wondering was, I always wonder how people make proxy website. So I post this thread today for HELP, how do I do this?

If you don't understand: Like websites you use to bypass other sites that are blocked at your school.
Something like this one: http://mathlol.com/

Login: beta
Pass: beta

Where is your hosting? If you are on shared hosting I don't recommended it. To my knowledge, you'll need a unique IP address to use as well. You can try these scripts, however:

Zelune - It uses Curl instead of PHP or CGI so make sure your host can use this

CGIProxy(recommended) - How to Install

There is a readme in both.
