I suggest you just be purely blunt about it xD
A simple "Do you want to be my girlfriend" won't matter to her if she really is into you.
I was in the same dilemma as you before, where I thought that was just bland and stupid. When the time came, I just blurted it out lol. Luckily it worked for me
Yeah, but I asked her how would she liked to asked out if someone did, she said "I want it to be creative."

I don't see why how you do it matters.. why cna't you just be blunt? I get she wants it to be 'creative', but the lines you have in mind are lame. I personally wouldn't worry about how to ask someone out.. I just would. If she doesn't like it, then .. does she really want to date you?
Well you got many tips here, but what I think is the most important;
1. Don't be shy.
2. Try to make her laugh.
3. Be natural, (NOT perhaps yourself, but be there on earth, not somewhere else.)
4. Make sure that she notices that you wanna be there!
Creative? Write a story. I've also been there. Tried that. That one didn't work well... ha..

You do realize if you're too blunt with the girl, she will feel pressured and almost always say "no," even if she does like you back.
