Why would his heart need to work harder?

because hearts aren't suppose to be that small, they can't build up enough blood pressure that way, so they need to work twice as hard to get the blood properly flowing.

Yeah most of the time really tall or short people or someone with weight problems die from heart failure and lung problems.
