vb.net, class diagrams

Hello guys does anyone know the following:

Good class diagram. Classes correct, relationships shown and largely correct, as are attributes. Inheritance used
Good object diagram.
Simple UI
Create Patients
Scroll through Patients
Shows all conditions and treatments for that patientCreate new Conditions and Treatment for a given patient, Can associate each treatment with a Therapist
Use of OO
Correct control structures and Data Storage

I am willing to pay for any good help!
