To the best of my knowledge operators take the calls, filter the important (non-prank) ones, and dispatch a police unit to the area of crime. The operators are not always 100% busy of course, if they were who would answer the phones if every operator was completely busy? I actually prank called the police (911) once when I was 8 years old and they came to my residence and gave me a one hour lecture on why I shouldn't joke with cops.
